Despite a substantial body of evidence that transgender women, assigned male at birth but self-identifying as women, are at high risk for HIV transmission, there remains a lack of insight into the burden of HIV among transgender women because of limited inclusion in national HIV surveillance systems. The Centers for Disease Control and Prevention reports that the percentage of transgender people who received a new HIV diagnosis is more than 3 times that of the national average. There is also a critical need for prioritized prevention and awareness programs targeted specifically to transgender women, as many are not aware of their HIV risk status and are not seeking effective prevention or screening care. This case vignette exemplifies the extent of these challenges and offers guidance on overcoming these barriers to care for an African American transgender woman at risk for HIV.
Primary care and internal medicine clinicians (MDs/DOs, physician assistants [PAs], nurses [RNs], and nurse practitioners [NPs])

Describe the role of HIV-prevention strategies in individuals at increased risk for infection
Counsel/educate at-risk individuals about HIV-prevention strategies
Employ HIV-prevention strategies for individuals at risk for HIV infection
Implement practical, evidence-based strategies to ensure that HIV-prevention services are delivered to patients in a culturally competent manner
