Program Description:

Most people with type 2 diabetes in the US are not at goal for glycemia and other comorbidities, including hypertension and dyslipidemia. This PI activity will provide physicians with the tools to assess their practice, choose appropriate educational and office system interventions, improve the care of their patients with diabetes, and measure the results.
